First you want to test the brake output circuit on the 7-Way Plug with a circuit tester like part # PTW2992 with the manual override or brakes applied in the vehicle. There was a large enough change between the 2016 and 2017 Ford F-350s that B and W had to come up with a new design for their gooseneck hitch fits. So for your 2012 Ford F-250 you'd need to go with the # BWGNRK1111 instead of what came out of the 2017 truck. view full answer...
For a plug that will fit the gooseneck ball hole in your 2011 Ford F-350 with the factory 5th wheel/gooseneck prep package you can use # RP30136. A trim ring for the gooseneck hole is not available.

I have a great solution that will allow you to tow a fifth wheel trailer with your 2009 Ford F-350 that has a B and W Turnoverball gooseneck hitch # BWGNRK1108 installed but the Reese rail kit you mentioned would not fit at the same time as your goosneck hitch. The issue you are having would only really be caused by an internal error in the brake controller. Therefore, the only option you have is a full replacement. It sounds like you might have a factory brake controller which I am not very familiar with but I do know they can be expensive to repair or replace so this might be a good time to upgrade to a quality aftermarket brake controller like the Tekonsha Primus IQ Trailer Brake Controller part # TK90160.
